MEDIA RELEASE & AUTHORIZATION AGREEMENT

I hereby grant EQUIP Ministries USA Inc. (“EQUIP Ministries”) and its authorized representatives

permission to photograph, videotape, audio record, or otherwise capture my image, likeness, voice,

statements, and participation during any EQUIP Ministries event, retreat, activity, or program.

I understand and agree that EQUIP Ministries may use, reproduce, edit, publish, display, distribute, and

otherwise utilize these photographs, videos, audio recordings, and other media for lawful ministry,

educational, promotional, advertising, fundraising, social media, website, and archival purposes, in any

format or medium now known or later developed.

I understand that I will not receive payment, royalties, or other compensation for the use of such media. I

waive any right to inspect or approve the final materials in which I may appear and release EQUIP Ministries

from claims arising from the authorized use of my image, likeness, or voice, except where prohibited by law.

This authorization is voluntary and remains effective for media captured during the participant’s

involvement in EQUIP Ministries activities unless revoked in writing. Any revocation will apply prospectively

and will not require EQUIP Ministries to retrieve or destroy materials already produced, published, or

distributed.

PARENT / LEGAL GUARDIAN CONSENT

Required for participants under 18 years of age

I am the parent or legal guardian of the above-named participant. I have read and understand this

agreement and hereby provide my permission and consent for EQUIP Ministries USA Inc. to photograph,

videotape, audio record, and otherwise use the participant’s image, likeness, voice, and statements as

described above.
